Compensation: $16-18 hourly
Location: Oklahoma City, Oklahoma 
Patient Care Coordinator 

Inceed has partnered with a great company to help find a skilled Patient Care Coordinator to join their team!

Responsibilities:

• Responsible for assessing finances for all new inquiries prior to admission, as well as current residents transitioning from skilled to long term care services.
• Responsible for data entry of resident’s income and assets into Financial Planning Questionnaire to assist in identifying possible Medicaid eligibility issues.
• Collecting required financial documents as identified during financial interview.

Required Qualifications & Experience:

• Highschool diploma or equivalent.
• Basic computer skills, including Microsoft Office.

Nice to Have Skills & Experience:

• Business Office Manager or Admissions Coordinator experience is preferred.
• Experience and knowledge of Long-term care Medicaid eligibility is preferred.
